Code Tenderloin provides Day Habilitation and Housing Transition Navigation to eligible Anthem Blue Cross Medi-Cal members in San Francisco. We offer whole person support including housing, employment, behavioral health, and daily living needs which are often connected. Our team coordinates services around each participant’s goals.
We work alongside ECM care managers, healthcare providers, housing teams, and other community partners without replacing or duplicating their services.
Day Habilitation
Day Habilitation helps you build practical skills for greater independence and stability.
Support may include:
Preparing for employment
Building healthy routines
Strengthening communication and social skills
Learning coping and self-management skills
Setting personal goals
Developing everyday independent-living skills
Our team works with you to understand your goals and create a plan that fits your needs.
Housing Transition Navigation
Housing Transition Navigation helps you prepare for and search for housing.
We can help you:
Understand your housing needs and options
Create a housing plan
Gather identification and other documents
Search for available housing
Complete housing applications
Communicate with landlords and housing programs
Connect with benefits and community resources
Housing Navigation does not guarantee housing, but our team will support you throughout the process.
